Coming home after a hospital stay or rehab is a relief — but the first few weeks are when falls and setbacks happen most. A little preparation goes a long way. Here’s a practical checklist Central Ohio families can use to make home safe before a loved one is discharged.
Start with the bathroom
The bathroom is the most common place for falls. Simple equipment makes a big difference: grab bars by the toilet and tub, a shower chair or transfer bench, a raised toilet seat, and non-slip mats. These are inexpensive and often the single best safety upgrade you can make.
Create a safe path to move
Clear walkways of throw rugs, cords, and clutter. Make sure the person has the right mobility aid for their strength and balance — a walker or rollator for support while walking, or a wheelchair for longer distances. Good lighting and a nightlight on the route to the bathroom help prevent nighttime falls.
Set up the bedroom for recovery
If getting in and out of bed is hard, a hospital bed with adjustable height and rails can make recovery safer and more comfortable — for the patient and the caregiver. A lift chair in the living room helps people stand without straining.
Don’t forget the supplies
Stock up on what you’ll actually use day to day: wound-care supplies if there’s an incision, incontinence supplies if needed, and any respiratory equipment your team recommended. Running out at 9 p.m. is stressful — we can help you plan ahead.
Ask about insurance early
Much of this equipment may be covered by Medicare, Medicaid, or your plan when it’s medically necessary — subject to eligibility and any prior authorization. Ask the discharge planner, and call us to check what applies.
